if it's a NG Nightmare, then yes, you need some luck to succeed. you could always spam some Adventure Mode Normal runs to get yourself some sweet gear. Nightmare isn't meant for brand new characters. it's meant for someone that's already gone through the entire campaign and has well-upgraded weapons/gear/mods.
